Place slides in a slide container, preferably wrapped in bubble wrap. Include either our requisition form or a physician’s consult request letter and the original pathology report, if available. Please also enclose a copy of the patient’s insurance card, if available. Place all items in a FedEx Clinical Pak bag, or a FedEx box. Adhere the FedEx airbill and choose Standard Overnight as the shipping method. Please call us toll free at 855-DERMPATH (855-337-6728) to obtain our FedEx account number for shipping. Alternatively, if you are located in the Shelby County or northern Mississippi area, our courier can pick up the slides: please call us 901-866-8834.
